Services may be covered in full or in part by your health insurance or employee benefit plan.
I am currently a provider for the following insurance companies:
BLUE CROSS BLUE SHIELD PLAN
COMPANION BENEFIT ALTERNATIVES
Most insurance companies providing health insurance benefits have mental health coverage as well
Please check your coverage carefully by asking the following questions
What is my mental health coverage?
- What is my deductible and has it been met?
- How many sessions per year does my insurance cover?
What is my copay or co-insurance responsibility per therapy session?
Is authorization required from my insurance company or a managed care company?
Am I eligible under an Employee Assistance Plan (EAP) through my employer or the employer of a family member?
An EAP may offer a specific number of sessions (for examples 4 or 6) at NO cost to you if you contact them directly for a referral.
I am a provider for numerous EAP programs. The need for therapy does NOT need to be work-related.
“Out of Network” If I am not a network provider for your insurance company, you may be eligible for “out of network” coverage if your policy provides this benefit.
“Self Pay” If you are not using insurance, there is a “self pay” option. The rate per appointment will be $77-$130.
Payment Cash, Check, Debit, MasterCard, Visa, Discover, American Express and HSA/FSA cards are accepted.
If you do not show up for your scheduled therapy appointment, and you have not notified the office at least 24 hours in advance, you will be required to pay a no-show fee.
